Card B pivots to now: acetaminophen is the #1 cause of acute liver failure in the US (56,000 ER visits/yr, ~500 deaths annually), FDA black-box warning current 2023, McNeil internal docs showed dosing confusion was known. Card C names the precedents: Bayer Aspirin (GI/Reye's risks understated for decades) and Phenacetin / APC tablets (sold as safe for 96 years, withdrawn as confirmed carcinogen 1983). Card B โ What Happened Next
Tylenol is the #1 cause of acute liver failure in the United States.
56,000 emergency room visits a year. Around 500 deaths annually from acetaminophen toxicity. The FDA's black-box warning on liver damage is current โ reaffirmed in 2023. Maximum 4g per day. Less if you drink. McNeil / J&J internal documents, surfaced in litigation, show the company was aware of consumer dosing confusion for years while Tylenol stayed on open shelves.
In 1998, the UK government didn't wait. Parliament restricted over-the-counter paracetamol pack sizes to reduce suicide-by-overdose incidents. A public-health decision, not a corporate one.
Tylenol is still the best-selling OTC pain reliever in America. It's in your bathroom right now.
Card C โ The Pattern
Bayer Aspirin, 1900sโ1950s. Marketed as universally safe for adults and children. GI bleeding risk: understated for decades. Reye's syndrome โ a potentially fatal condition in children who take aspirin during a viral illness โ wasn't fully characterized until the 1980s. UK and US eventually restricted pediatric aspirin use.
Phenacetin, 1887โ1983. Nearly a century of "it's the safe alternative." Withdrawn only after confirmed as a carcinogen and nephrotoxin โ kidney damage, bladder cancer. Sold in APC tablets (Aspirin + Phenacetin + Caffeine) marketed as the miracle headache cure.
The safe pill changes. The reassurance never does.
